The Center for Developmental Services (CDS) in Greenville, SC has expanded its case management program and is now accepting new referrals for the first time in five years. This expansion is made possible through support from the Greenville Health Authority’s Healthy Greenville grant initiative, aimed at improving access to disability services for eligible individuals with the SC Department of Disabilities and Special Needs.
CDS’s case management program has consistently been recognized as one of the top providers in the state, receiving high quality scores for their coordination of disability services. With this program expansion, CDS aims to reach more individuals and families in need of comprehensive case management services.
Case management at CDS involves creating a personalized care plan for individuals with disabilities, considering their specific needs and goals in collaboration with them, their family members, and other professionals. This plan covers areas such as educational goals, skill development, in-home support, employment resources, housing solutions, medical needs, and equipment. Case managers also offer ongoing support to clients as they navigate life transitions.

CDS is the largest multi-disciplinary treatment facility in the Upstate for children with disabilities and developmental delays, offering a range of services such as therapy, diagnosis, audiology, daycare for medically fragile children, early intervention, case management, and a family support network. Partnering with organizations like Clarity, Project Hope Foundation, KidVentures of Thrive Upstate, and programs of Prisma Health – Upstate, CDS serves over 8,000 children and their families annually.
The expansion of CDS’s case management program will provide essential support to individuals and families navigating the complex disability services system. Through comprehensive care coordination, CDS aims to enhance access to necessary resources and help individuals with disabilities reach their full potential.
